Job Type: Contract (15 months), Potential for conversion

Compensation: $32.00-52.00/hr

Our client is a leading nonprofit health insurance and healthcare services organization serving members across the Upper Midwest. They are committed to improving health outcomes through coordinated, member-centered care and innovative healthcare solutions.

We are seeking an experienced Behavioral Health Case Manager to join a remote care management team. In this role, you will support individuals with complex behavioral health needs by coordinating care, developing individualized care plans, and helping members navigate the healthcare system. This position offers the opportunity to make a meaningful impact on patient outcomes while working collaboratively with members, families, providers, and interdisciplinary care teams.

This is a 100% remote, telephonic position with no in-person member meetings required. Candidates residing in Wisconsin, particularly those familiar with community resources in and around Dane County, are strongly preferred.

Job Description

The Behavioral Health Case Manager is responsible for providing comprehensive case management services for members with complex behavioral health needs. Using an evidence-based and member-centered approach, the Case Manager works to improve quality of life, optimize healthcare outcomes, and support appropriate utilization of healthcare resources.

Key Responsibilities

  • Conduct comprehensive behavioral health assessments to identify member needs and care gaps.
  • Develop, implement, and monitor individualized care plans aligned with established treatment goals.
  • Coordinate services across the healthcare continuum, including behavioral health, medical, and community-based resources.
  • Collaborate with members, families, caregivers, providers, and interdisciplinary care teams to support holistic care delivery.
  • Advocate for members and assist them in navigating healthcare and social service systems.
  • Monitor member progress and adjust care plans as needed to support positive health outcomes.
  • Support efforts to reduce illness burden, improve member well-being, and manage healthcare costs.
  • Maintain accurate documentation and manage case activities using multiple technology platforms and systems.
  • Meet established departmental performance metrics and quality standards.

Qualifications

Required

  • One of the following:
    • Associate's or Bachelor's degree in Nursing, or
    • Master's degree in Social Work with advanced practice licensure.
  • Current, unrestricted RN license in state of residence or current, unrestricted advanced practice Social Work license in state of residence.
  • Minimum of 5 years of clinical nursing or social work experience with a focus on behavioral health.
  • Experience working with vulnerable and complex populations in clinical, home care, telephonic, or similar healthcare settings.
  • Strong understanding of behavioral health conditions and care management principles.
  • Knowledge of healthcare systems and services across the continuum of care.
  • Ability to effectively manage multiple priorities and complex cases in a fast-paced environment.
  • Strong analytical, problem-solving, and clinical decision-making skills.
  • Excellent verbal and written communication skills.
  • Proficiency using multiple computer systems, applications, and care management tools.
  • Ability to work independently in a remote environment while maintaining high levels of collaboration and customer service.
Preferred

  • Direct case management experience.
  • Clinical social work licensure.
  • Certified Case Manager (CCM) credential.
  • Familiarity with community resources in Wisconsin, particularly Dane County.
  • Experience supporting diverse populations across varying age groups, cultural backgrounds, and socioeconomic circumstances.
  • Experience in managed care, health insurance, or telephonic care management settings.
  • Demonstrated ability to lead change initiatives and influence positive team outcomes.
