Position Summary
The Case Manager at Freedom Behavioral Lake Charles plays a vital role in supporting patients throughout their treatment journey by coordinating care and ensuring each patient receives the services, resources, and support needed for successful recovery and continuity of care.
In this role, the Case Manager conducts assessments, assists with individualized treatment and discharge planning, and collaborates closely with physicians, nurses, therapists, families, and community partners to address each patient’s mental health and social needs. Responsibilities also include coordinating follow-up services, connecting patients with community resources, and supporting safe and effective discharge planning.
The Case Manager serves as an advocate and key point of communication between patients, families, and the treatment team while helping promote positive patient outcomes and stability. This position is ideal for a compassionate, organized, and motivated professional with strong communication skills and a passion for helping others.
Preferred Knowledge, Skills, And/Or Experience Required
Education: A psychology major is preferred or nursing education
License: Nursing License, Social Worker License, &/or LPN
Experience: 1+ years coordinating utilization review and discharge planning activities
