We are a patient-centric, technology-enabled healthcare organization that helps our partners deliver proactive, coordinated care through programs like Chronic Care Management, and other population-health programs.
About The Team You'll Join
We're a company of thinkers, doers, problem-solvers, builders, and patient advocates. People who see a gap and close it, not wait for someone else to.
- You'll work cross-functionally, connected to a mission that touches clinical operations, technology, and patient care at every turn.
- At Axil Health, you join a small team where your work is visible, your voice counts, and your judgment shapes how things get built, not just how they run.
The Strongest Fits Tend To Be People Who
- Care about the patient behind the data point. Whether you're in clinical operations, care navigation, or support functions, the work touches real people managing real health conditions.
- Are comfortable with ambiguity and building, not just maintaining. We're still defining processes in many areas, so people who can bring structure without waiting to be told exactly how do well.
- Communicate directly and kindly. We value people who raise issues early, give and receive feedback without drama, and treat colleagues with respect even under pressure.
- Take ownership without needing constant oversight, self-direction matters as much as collaboration.
- Stay curious. Healthcare and patient monitoring technology both evolve quickly, and people who keep learning keep up.
- Make a tangible difference in patients' lives every single day
- Work with a passionate team that feels like family
- Gain experience with cutting-edge healthcare technologies
- Enjoy a supportive work environment that encourages professional growth

The Manager of Practice Success & Transformation is the primary operational relationship owner for a portfolio of Axil Health physician-practice and health-system partners. This individual works directly with practice administrators, office managers, physicians, providers and Axil clinical teams to ensure each partnership is successfully implemented, consistently supported and producing measurable clinical, operational and financial value.
The ideal candidate understands how medical practices operate, can earn credibility with physicians and practice leaders, and is comfortable using performance data to identify problems, facilitate decisions and drive accountability.
This is not a traditional customer-support position. It is a practice-facing leadership role responsible for partnership success, workflow transformation, program adoption, performance improvement, retention and responsible growth.
