We're building a connected AI operating platform for outpatient healthcare organizations.

We're looking for a hands-on CTO who will write and review production code, make critical architecture decisions, and build the engineering team and operating systems needed to scale.

What we're building

  • Aria Admin - voice AI receptionist handling inbound calls, scheduling, and intake. Live with pilot practices.
  • Aria Chart - clinical documentation that produces notes clinicians can sign without rewriting.
  • Aria Audit - pre-bill audit that identifies coding and documentation gaps before claims are submitted.
  • Aria Care - patient support between visits and at home, guided by the patient's treatment plan.

These products are designed to share one governed platform. These products operate on a shared platform and patient context. Aria ADMIN supports patient access and intake, Aria Chart supports clinical documentation, Aria Audit reviews documentation before billing, and Aria Care supports patients between visits. The CTO will unify and scale these capabilities into a reliable, secure platform for healthcare organizations.

The role

This is a hands-on technology leadership role. The CTO will remain actively involved in architecture, code review, critical production systems, and engineering delivery while building a capable and accountable team.

What you'll own

  • Technical strategy and architecture for a secure, multi-tenant healthcare AI platform, including clear boundaries between shared platform services and product-specific workflows.
  • Hands-on delivery of critical systems, including writing production code, reviewing pull requests, resolving difficult technical problems, and improving engineering quality.
  • Production AI systems, including retrieval, tool use, structured outputs, evaluation, confidence thresholds, human escalation, model selection, and cost and latency management.
  • Reliable voice, clinical, and workflow automation that integrates safely with external healthcare and practice-management systems.
  • Security, privacy, reliability, observability, incident response, and the technical controls needed for enterprise healthcare customers.
  • Engineering execution: priorities, delivery cadence, technical debt decisions, documentation, on-call practices, and measurable service ownership.
  • Team design, hiring, coaching, and clear accountability across a growing engineering organization.

What we're looking for

  • 10+ years building software, with recent, meaningful experience as a hands-on senior, staff, principal, or founding engineer - not only managing engineers.
  • Experience taking at least one product from prototype to production at meaningful scale, with direct responsibility for reliability and incidents.
  • Strong backend and platform engineering judgment across APIs, databases, multi-tenant systems, event-driven workflows, integrations, and cloud infrastructure.
  • Production depth in at least two of the following: real-time voice or audio systems, applied LLM systems, healthcare data and integrations, or mobile and web product delivery.
  • A practical understanding of where AI is dependable, where safeguards or human review are required, and how to measure system quality over time.
  • Evidence of building and leading strong teams while maintaining appropriate technical involvement.
  • Strong product and business judgment, including the ability to translate customer problems into focused technical priorities and make sound build, buy, and sequencing decisions.

Preferred experience

  • TypeScript or Node.js, Python, PostgreSQL, APIs, queues, and event-driven services.
  • AWS or another major cloud platform, infrastructure as code, containers, CI/CD, monitoring, and secure production operations.
  • EHR and practice-management integrations, FHIR or HL7, scheduling, patient matching, eligibility, clearinghouses, webhooks, and healthcare data normalization.
  • HIPAA-regulated products and enterprise security or assurance programs such as SOC 2 or HITRUST.
  • AI-assisted software-development tools used with disciplined review, testing, and security practices.

How to apply

Send a note to [email protected] describing a production system you helped build, your direct contribution, one thing you would do differently, and a difficult production incident you helped resolve. Please also confirm whether you are considering a full-time operating role or a fractional or advisory commitment. Do not send confidential code or proprietary material. Skip the cover letter.
