What We’re Looking For

Trillium Health Resources has a career opening for an Innovations Waiver Regional Manager to join our team! The Innovations Waiver Regional Manager oversees a skilled, integrated team of professionals who provide localized support within a managed care environment for members living with intellectual/developmental disabilities or traumatic brain injury. The Innovations Waiver Regional Manager ensures staff implement industry standards, evidenced-based and best practices to support members. The Innovations Waiver Regional Manager provides oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better.

On a typical day, you might:

  • Provide direct supervision and administrative oversight for a localized, Integrated Care Team supporting Innovations members and members without Innovation waiver funding living with mental health, substance use, and intellectual/developmental disabilities.
  • Ensure staff utilize workflows as required to complete Care Management Comprehensive Assessment and care plans for members as care needs change.
  • Monitor teams to help ensure integration of care through the establishment of an integrated care team (e.g. member, caretaker/legal guardian, family member, PCP, behavioral health provider, specialists, nutritionists, pharmacy, etc.) to meet the member’s needs.
  • rack the implementation of workflows, policies, and procedures that screen for and track on members’ social determinants of health needs that can include food, housing, transportation, and interpersonal safety/toxic stress. Confirm that team members are able to identify needs, locate community resources, and link members to resources that help to address their social determinants of health needs.
  • Establish objective, quantitative performance measures to ensure that the team’s activities are efficient and help to drive member outcomes in addition the quality measures and outcomes set for the organization.

Qualifications

Required:

  • Bachelor’s degree in Health, psychology, sociology, social work, nursing, or another relevant human services area and a minimum of one (1) year of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI; OR
  • Associate’s degree in Health, psychology, sociology, social work, nursing, or another relevant human services area and a minimum of three (3) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI; OR
  • Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within Mid State Region, which includes the following counties: Anson, Guilford, Montgomery, Randolph, and Richmond.
  • Must be able to travel within catchment as required.

Preferred:

  • Supervision of two (2) or more employees
  • Experience focused on working with Innovations in the mental health, substance use, and/or intellectual/developmental disability and TBI populations
