Admissions Coordinator Duties and Responsibilities:
- Responds to calls regarding information, admission to a facility and re-admission.
- Secures information on the needs of the customer, identifies services available to meet the needs of the customer and secures admission to the facility.
- Secures required information (current medications, labs, history and physical, diagnosis, current notes) to finalize the acceptance of a new admission or re-admission of a current resident/patient.
- Communicates with referral sources, prospective admissions, and the facility, in an expeditious manner, on the acceptance (or denial) for an admission and planned admission date. (Partners with the designated lead prior to denying an admission.)
- Communicates/partners with the financial lead on the key elements relating to an admission, such as but not limited to insurance verification, OBRA screening, Medicare qualifying stay, etc.
- Communicates with facility staff to ensure accuracy of information relating to bed availability, planned discharges, residents/patients needing isolation, transfers to the hospital and deaths.
- Meets with facility staff to develop partnerships, knowledge of and observation of the key services as well as evaluation of the admission and re-admission process (seeks information from the marketing staff for the evaluation).
- Meets with referring agencies to develop partnerships, knowledge of the needs of the referring agencies as well as evaluation of the admission and re-admission processes (seeks information from the hospital liaison for the evaluation
- Evaluates the admission process and develops measures for improvement in conjunction with the key facility partners.
- Secures information relating to the referring agency and/or call-how they learned about the facility, name of referral, etc. and analyzes this information with the marketing staff.
- Coordinates with the hospital liaison for on-site assessments.
- Coordinates with the DON for medical review of admissions with complex medical care requirements.
- Performs duties, which may include transportation of residents, as assigned in Facility
- Performs other duties, as assigned.
- Ability to read, write, speak and understand English.
- Meets all health requirements, as required by law.
- Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.
- Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
- High school degree or equivalent, required.
- Bachelor’s Degree in an area relating to health, human services or customer service preferred.
- One year of experience in a related role, preferred.
- Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
- Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
