The RN Care Manager position requires a thorough understanding of clinical, financial, administrative, and psychosocial components of the healthcare continuum The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members across the continuum of care. These services are provided face to face or telephonically to facilitate the creation of and adherence to, a treatment plan and optimize patient health. In conjunction with the interdisciplinary team, this position proactively addresses potential or actual clinical and/or psychosocial needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated. Shriners Salt Lake City is looking for a full time RN Care Manager to join our team!
Schedule Full-time 1.0 FTE (In Person)
Holidays 9 observed holidays per year
*The pay range for this position is $34.90/hr. - $52.35/hr. Compensation is determined based on years of experience and departmental equity.*
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the SC system to jointly communicate, problem solve, and share accountability for optimal outcomes
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care
- Advocates by supporting or recommending services for the protection of the patient's health safety and rights
- Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to assure appropriate Resource Management
Required
- Bachelor of Science in Nursing
- Current Utah RN license
- Case Management Certification (CMC, ACM or ANCC) within 2 years of hire
- 1 year Care Management in Pediatrics
- 3 years clinical nursing experience-pediatrics preferred
- Bilingual
