Position Overview
Concierge Home Care is hiring a Hospital Liaison (RN or LPN) to serve as the connection between hospital teams, physicians, patients, and home health services. This role bridges the gaps in the transitional care and focuses on:
- Building referral relationships
- Supporting smooth hospital-to-home transitions
- Educating patients and families
- Coordinating post-discharge care
- Driving admissions growth within the assigned hospital systems
Compensation & Schedule
- Earn $75K+ annually

- Coordinate care from hospital to home
- Collaborate with:
- DME providers
- Infusion companies
- PCPs
- Caregivers
- Support patients during critical post-discharge periods
- Help reduce avoidable readmissions
- Partner closely with:
- Intake teams
- Clinical managers
- Schedulers
- Account Executives
- Provide timely referral and patient updates
- Ensure continuity of care throughout the admission process
- Process referrals in real time
- Complete admission documentation accurately and in a timely manner
- Track:
- Referral activity
- Conversion rates
- Admission barriers
- Active Florida RN or LPN license
- Strong communication and interpersonal skills
- Detail-oriented and organized
- Microsoft Office proficiency
- Valid driver’s license and reliable transportation
- Auto insurance
- Home health experience
- Hospital liaison or healthcare marketing experience
- Case management or transitional care experience
- Knowledge of payer sources and referral processes
