- To undertake delegated duties autonomously following assessment and direction from the Health Visitor (HV) caseload holder, delivering agreed objectives within clients’ homes and community settings.
To screen and monitor the growth and developmental progress of pre‑school children in line with the Healthy Child Programme (HCP), using evidence‑based assessment tools.
To deliver evidence‑based group programmes for parents and children under professional supervision, ensuring fidelity to the specified programme models and intervention frameworks.
To practise in accordance with the Stockport Family Model of restorative practice, ensuring assessments and interventions are undertaken collaboratively with families.
To contribute to and support supervised contact sessions alongside the allocated contact supervisor, where required for families on the caseload.
To undertake home visits independently or jointly with colleagues, as appropriate.
To undertake any additional duties appropriate to the role, as agreed in consultation with the line manager.
The post holder will work as an integral member of the Stockport Family Service, supporting the health, learning, and developmental outcomes of children through a range of targeted interventions and strategies delivered within home and community settings. These include Start Well Hubs, Satellite Hubs, Health Centres, and Early Years settings such as nurseries.
The post holder will work collaboratively with partner agencies to ensure services are well‑coordinated and that groups and activities are responsive to the needs of children under five and their families.
