CardioRenal Vision is a multi-state specialty medical group providing on-site cardiology, nephrology, and pulmonology services to post-acute care facilities.

Our mission is to improve patient outcomes, reduce hospital readmissions, and support facility care teams through collaborative specialty care delivery.

Position Overview

CardioRenal Vision is seeking a motivated and relationship-driven Business Development Specialist to support growth initiatives and strengthen clinical partnerships within the post-acute care market.

This role combines healthcare relationship management with strategic outreach, helping facilities identify appropriate patients for specialty consultations while fostering long-term partnerships with facility leadership and care teams.

Key Responsibilities
  • Develop and maintain strong relationships with facility leadership, nursing staff, case managers, and care coordinators
  • Educate partner facilities on CardioRenal Vision’s specialty services, workflows, and consultation process
  • Assist facilities in identifying appropriate patients for cardiology, nephrology, and pulmonology consults
  • Support onboarding of new facilities and providers, including workflow training and implementation
  • Collaborate with providers and internal operations teams to ensure seamless care coordination
  • Attend facility meetings, in-services, and occasional on-site visits
  • Track outreach efforts and contribute to growth initiatives within assigned territories
Qualifications
  • Clinical experience in SNF, LTACH, post-acute care, hospital discharge planning, or case management preferred
  • Existing relationships within hospitals and skilled nursing facilities strongly preferred
  • Excellent communication and relationship-building skills
  • Comfortable presenting to both clinical and administrative teams
  • Self-motivated, organized, and able to work independently
  • Experience in healthcare business development, physician liaison, or clinical outreach is a plus
