Founding Care Manager (Hybrid Remote + In-Home Senior Support)

Seniors at Home is a growing healthcare and aging-in-place practice based in Bend, Oregon. We help seniors remain independent, safe, and connected at home through proactive care management, chronic care coordination, remote monitoring, and in-home support.

We are looking for a high-performing, emotionally intelligent Care Manager to become one of the foundational members of our team. This is not a traditional clinic role.

This role is ideal for someone who:

You will work closely with providers, therapists, patients, and families to coordinate care, support aging in place, and create an exceptional patient experience.

What You’ll Do

• Coordinate ongoing care for seniors enrolled in our programs
• Conduct regular phone and occasional in-home check-ins
• Communicate with patients, families, providers, and community partners
• Support care plan follow-through and appointment coordination
• Document patient interactions and care activities accurately
• Identify changes in patient condition or emerging concerns
• Help improve workflows and operational processes as we grow
• Deliver a warm, professional, high-trust experience to every family

Compensation

Expected range:

$25–$36/hour DOE
