Breezy Hills Rehabilitation and Care Center is seeking a motivated and compassionate Social Services Assistant to support our residents, families, and care team. In this role, you’ll help create a positive and supportive environment, ensuring the emotional, social, and psychological needs of our residents are met.

Key Responsibilities:

• Assist with the daily operations of the Social Services Department in compliance with federal, state, and facility regulations.
• Support discharge planning, care plan development, and resident assessments.
• Collaborate with administration, nursing, and medical staff to address resident needs.
• Encourage resident and family participation in care plans and meetings.
• Promote Residents’ Rights, dignity, and respect among staff and residents.
• Follow infection control protocols and report exposures promptly.
• Participate in safety and emergency preparedness training.
• Maintain CPR/First Aid certification and complete required ongoing training.

Qualifications:

• High school diploma or equivalent required.
• Minimum 1 year of experience in a nursing home or healthcare setting preferred (but not required).
• Experience in an office setting with strong administrative and organizational skills.
• Strong communication skills in English (both verbal and written).
• Empathy, compassion, and professionalism when working with elderly residents and their families.
• Ability to stay calm under pressure and work effectively as part of a team.
• Basic computer skills.
