Responsibilities

• Conduct psychosocial assessments for adult and geriatric patients and develop the plan of care in collaboration with the interdisciplinary team
• Coordinate discharge planning and care transitions, including referrals to community resources and services
• Document assessment findings, care plans, and progress notes in the EPIC charting system
• Support patients and families in navigating medical, emotional, and social needs during acute care and discharge planning
• Collaborate with social services, physical therapy, pharmacy, interpretation services, transportation, and other team members to optimize patient outcomes
• Float across hospital units as needed within the two campus locations to meet patient care demands

Requirements

• Active Washington state social work license in good standing (LICSW, LSWAIC; LMSW, LCSW; Pending license accepted per posting; WA license required)
• Certification: BLS required
• Experience: Minimum 1 year of clinical social work experience; posting notes also reference a 2-year minimum in some instances—please confirm with the driving account manager
• EPIC charting experience required
• Ability to work with adult and geriatric populations in acute care, discharge planning, and care coordination settings
• Travel assignment: Locals not accepted; 50-mile radius rules apply
• Orientation and onboarding: 40 hours of orientation prior to start; pre-start modules may be required
