Role Description

This is a part-time, on-site role for a Home Health Nurse based in Youngstown, OH. The Home Health Nurse will focus on delivering patient-centered care, including administering treatments, performing wound care, providing any skills related to G-Tube, Tracheostomy, or Ventilator care, monitoring patient health, and providing detailed documentation. The role includes coordinating with healthcare providers to ensure optimal outcomes and assisting with home care tasks to support overall patient well-being.

Qualifications

• Proficiency in Nursing and Medicine with a current RN or LPN license
• Must have at least a year's experience in Patient Care, particularly in supporting individuals with diverse health needs
• Skills in Wound Care and the ability to manage treatment plans effectively
• Strong communication and organizational skills
• Ability to work independently and collaborate in a team-oriented environment
• CPR and BLS certification (Required)
• Negative TB test (Required)
• Reliable transportation and the ability to travel to patient homes
