The Pharmacy Specialist responsibilities include, but are not limited to, the following:
- Provides concise, applicable, and timely responses to requests for drug information from healthcare professionals affiliated with INTEGRIS Health
- Identifies drug use issues requiring policy development
- Coordinates the concurrent drug use evaluation program involving the staff pharmacists
- Assists in the assessing, managing, preventing, and reporting of adverse drug events
- Tracks and trends ADRs for potential educational opportunities/drug policy issues involving medication use
- Prepares and presents drug and therapeutic class monographs for formulary review by the Pharmacy and Therapeutics Subcommittee
- Provides in-service education to the healthcare professionals of INTEGRIS Health; develops and distributes a drug use management oriented newsletter
- Oversee the formulary approved medications and provide for the availability of these formulary agents to the respective institution Comprehensive Medication Management Only
- Elicits appropriate medication history from patient and reconciles medication list.
- Assesses medication regimen for efficacy, safety, clinically-relevant drug interactions, medication adherence, and costs of therapy.
- Recognizes effects of socioeconomic, behavioral, psychological, and environmental factors of patient adherence and counsels patients regarding their influence on the patients individual medication regimen.
- Addresses medication-related problems through patient education, medication changes per CMM scope of practice and protocols, or recommendations to primary care physician or specialist as situation warrants
- Provides patient-centered care that takes into account the patient preferences and insurance formulary in selecting drug therapy.
- Identifies and triages medical problems by referring patients to appropriate health care provider.
- Communicates clearly and appropriately with patients, physicians, care coordinators, and other health care team members.
- Effectively educates patients and caregivers on medications
- Documents all CMM visits in the electronic health record in a timely manner.

Qualifications
REQUIRED QUALIFICATIONS
EXPERIENCE:
- Post graduate year one (PGY1) residency or equivalent experience
- Graduation from a program in Pharmacy from an accredited school of pharmacy
- Advanced degree in pharmacy and/or a pharmacy residency or fellowship or equivalent experience
- Current Oklahoma Pharmacy license or For Pharmacists licensed in another state, a grace period of 45 days from hire is allowed to complete licensure process and present active Oklahoma Pharmacist License. During the grace period, no duties may be performed that require a license
- Must be able to communicate effectively in English (verbal/written).
- If specialty residency training available in scope of practice, specialty residency or fellowship in practice area
- Board certification in practice area specialty if available or general pharmaceutical certification (BCPS) preferred within one year of hire
