This role sits at the intersection of healthcare strategy, population health innovation, and mission-driven partnership development, focused on improving outcomes for individuals living with chronic kidney disease. You will engage directly with senior leaders across health systems, payers, and public health organizations to position kidney care as a strategic priority and drive large-scale, data-informed initiatives. The position blends external relationship-building with program design, implementation, and impact evaluation across regional and national levels. You will play a key role in translating clinical and population health data into actionable strategies that advance equity and improve care delivery. Operating in a highly collaborative, remote-first environment, you will also support internal teams in strengthening partnership capabilities and scaling successful programs. This is a high-impact role where strategy, communication, and healthcare expertise converge to influence outcomes at a national level.
Accountabilities:
In this role, you will lead the development of strategic partnerships and population health programs aimed at improving kidney care outcomes and advancing health equity.
- Build and manage partnerships with health systems, payers, public health agencies, and community organizations to advance population health initiatives.
- Engage senior clinical and executive stakeholders to position kidney disease as a strategic priority across healthcare systems.
- Design, implement, and evaluate population health programs, including CKD detection and management initiatives.
- Use healthcare data (claims, outcomes, and quality metrics) to identify opportunities, measure impact, and guide program improvements.
- Collaborate with internal teams to develop grant proposals and secure funding from government and foundation sources.
- Lead program quality assurance efforts and communicate results to both internal and external stakeholders.
- Support organizational capability-building by advising teams on population health strategy and partnership development.
- Master’s degree in public health, health policy, health sciences, or equivalent professional experience.
- 5+ years of experience in population health, healthcare partnerships, health systems, payers, or public health programs.
- Strong understanding of healthcare data, including claims, quality metrics, and outcomes measurement frameworks.
- Demonstrated ability to engage and influence senior clinical and executive stakeholders.
- Deep knowledge of health equity, social determinants of health, and disparity reduction strategies.
- Excellent communication skills, with the ability to translate complex data for diverse audiences.
- Experience working in remote, matrixed, and cross-functional environments.
- Experience with grant writing, value-based care, or integrated delivery systems is a plus.
- CPHQ certification and bilingual English/Spanish skills are considered advantageous.
- Salary range: $76,800 – $153,600 annually
